Document A3 - Assignment details form: Limited company contractors

You can use this form when supplying limited company contractors who are agency worker for the purpose of the Agency Workers Regulations 2010.

You should use this form with the following REC documents: 

• Contract 5 - Private Sector (Inside IR35): Opted out of the Conduct Regulations (For the Client)
• Contract 6 - Private Sector (Inside IR35): Opted out of the Conduct Regulations (For the Limited Company)
• Document G - Information request form: Agency workers

OR

• Contract 7 - Private Sector (Inside IR35): Not opted out of the Conduct Regulations (For the Client)
• Contract 8 - Private Sector (Inside IR35): Not opted out of the Conduct Regulations (For the Limited Company)
