Document J – Information request form - ‘Swedish Derogation’ contract

You can use this form when you supply temporary worker using contract 14, the ‘Swedish Derogation’ contract. 
 You can use this form to obtain from your client about the terms and conditions that they would apply to your temporary worker, if they had recruited the temporary worker directly. This will help you to comply with your obligations under the Agency Workers Regulations 2010.
