Document G - Information request form: Agency workers

You should use this form when you are supplying temporary workers who are agency workers for the purpose of the Agency Workers Regulations 2010 (AWR).

This form will guide you to collect the information that you need from your client to help you comply with the AWR.

This can be used with the following REC documents:

• Contract 3 - Terms of business with a hirer for the supply of agency workers
• Contract 4 - Terms of engagement for agency workers (contract for services)
• Contract 5 - Private Sector (Inside IR35): Opted out of the Conduct Regulations (For the Client)
• Contract 6 - Private Sector (Inside IR35): Opted out of the Conduct Regulations (For the Limited Company)
• Contract 7 - Private Sector (Inside IR35): Not opted out of the Conduct Regulations (For the Client)
• Contract 8 - Private Sector (Inside IR35): Not opted out of the Conduct Regulations (For the Limited Company)
