Document H - Assignment details form (to give to the employee)
MS WordYou MUST use this assignment details form when you are using Contract 14, the ‘Swedish Derogation’ contract for your temporary workers.
This assignment details form together with the contract 14 Agreement makes up the entire contract of employment. If this form is not issued to the Employee the employing agency will not be compliant with Regulation 10 of the Agency Workers Regulations, meaning that the obligation to provide equal treatment in regards to pay will still apply.
This form can be used with the following REC documents:
- Contract 13 - Terms of business with a hirer for the supply of agency workers engaged by the employment business under a Regulation 10 compliant contract of employment (updated January 2019)
- Contract 14 - Terms and conditions of employment (Regulation 10) (updated January 2019)
- Document I – Assignment details form (to give to the client)
- Document J – Information request form: ‘Swedish Derogation’ contract
- Document K - 48 hour opt out agreement for employees
