48 hour opt out agreement - for agency workers

This agreement is for to use for your agency workers if you want to obtain their agreement to work beyond the 48hr working time limit. If your worker has not agreed to this they must not be required to work in excess of 48 hrs which is averaged over a 17 week reference period.
